Registration Form

To enroll in EdBoost, please complete a registration form.  This form not only collects important information about your child and your family, but asks you to acknowledge our rules and policies. You are only required to initial the declarations that apply to the programs in which your student is enrolling. 

The form will ask you to acknowledge that you have read our Policies & Procedures.  That document is linked below.
